Job Description JOB SUMMARY: Manages patient transitions across the care continuum and serves as a facilitator of interdisciplinary collaboration and the point person for the patient from pre admission/admission to return to patient's medical home.
MINIMUM QUALIFICATIONS: Master's degree in nursing. Advanced Practice Nurse or Clinical Nurse Specialist perferred. Must be currently licensed as a RN in the state of Georgia. A minimum of two years experience of acute inpatient or critical care experience. In-depth knowledge of service specific disease pathophysiology. Nurse Practitioners must be authorized as an Allied Health Professional under the supervision of a sponsoring physician medical staff member. Must be a strong collaborator and facilitator of interdisciplinary teams.
